Registration and Transcripts

Welcome to CONCORD HIGH SCHOOL! I am Jane Kwiatkowski, REGISTRAR at Concord High School.


If you have input info via AIR please note that this does NOT complete the registration process at all; please follow the info below and CALL for an appointment; you will need to provide the list of documents at the appointment; AIR does not register your student.


If you have just moved into the area, please note that there are five comprehensive high schools within MDUSD and ALL enrollments are based on your home address. Please check your new address at; go to the SCHOOLS drop down, clock on it then go to SCHOOL FINDER, click on it and input your address for the info.


If your address IS in the CHS attendance area, then please call to schedule an appointment, as enrollment is by appointment only. The following documents MUST be presented at the appointment and are the responsibility of the parent/guardian to provide:

  1. Address verification:  Parents ID; current valid CA car registration; current lease or paystub; if you just purchased, close of escrow papers. ALL documents must show the correct home address.
  2. Student’s Birth Certificate.
  3. Student’s Immunization Records; MUST include Tdap booster shot (given after 7thgrade) and if coming from out of state, Varicella shot. These requirements are California State Law. Additionally, there are NO Personal Beliefs exemptions as of 1/1/2016 per CA State law-No shots, No School.
  4. Current transcript, drop grades, 504 (if applicable), IEP (if applicable). If you are enrolling an incoming 9th grader, current 8th grade report card.

Please remember, enrollment is BY APPOINTMENT ONLY and you need ALL documents with you to enroll your student. My phone number is 925-682-8000, ext. 3311. And please note my office is open year round!

Students may take select courses from the local community colleges, while concurrently enrolled at CHS. Students must complete page 2 of the applicable college form below, then submit to the Registrar for approval by the Principal. Allow 48 hours turnaround to get the approved form back before enrolling at the college. Additionally, the colleges may also have other requirements.

Are you a former Graduate of CHS and need a transcript?


Please come in to CHS M-F, 7 to 4, and fill out the form for this that our front office Secretary, Brenda, can provide you.


There is a $5 fee; cash or money order OR pay with debit or credit between the hours of 8:30 and 4:00.


Or call me at 925-682-8000, ext. 3311 and leave your name, spell out both first and last names, DOB, year you graduated, reason you need the transcript and your contact number. Either way, the turnaround time is 48 hours (not counting weekends).
