Want to play a sport at chs?
Each student and parent/guardian MUST complete the SportNet Online Athletic Packet PRIOR to the student trying out for a sport. Click on the links below for instructions on how to register your student and to print out the physical examination form before going to the doctor. You must "edit" and update the online form each year if you have previously logged in to SportsNet.

Transfer Information
If you have transferred into CHS from another school OR have moved into the area AND you want to play a sport, you must make an appointment to speak with the Athletic Director. Until the proper paperwork is filled out and approved by North Coast Section (NCS), all transfers are initially considered ineligible and can not compete until NCS clears them.

Parent Driver Form
Transportation of Students in Privately-Owned Vehicles
If you are planning on being a parent driver for any of the athletic teams you must:
1) Get fingerprinted through MDUSD and provide a current TB test to MDUSD. Appointment information available here.
2) Have the correct insurance to drive. Please note that the district requirements are higher than some basic insurance minimums.
